Through the Affordable Care Act, the Health Insurance Marketplace was created to assist individuals in finding affordable health insurance coverage. CPHD2 offers free enrollment assistance to patients and community residents. Our Certified Application Counselors are available to assist both new and renewing applicants. This service is free and open to all.
The 2025 Health Insurance Marketplace Open Enrollment begins Nov. 1, 2024 and continues through June 31, 2025. Click here to browse through 2025 plans and prices.

Medicaid is the state's health coverage program for low-income residents who meet certain eligibility qualifications. And, under Louisiana's recent Medicaid expansion, many more will now be eligible for Medicaid. As a Certified Medicaid Application Center, we can assist you with enrollment and/or answer any questions you may have about your current Medicaid plan.

CPHD2 offers a sliding scale fee based on your income if you do not have health insurance. But why not get insured? We have insurance specialists who will work with you one on one to get you enrolled into a health plan that meets your budget and needs.

What to bring to your appointment
If you are meeting with one of our Certified Application Counselors to apply for insurance, please bring the following information to your appointment:
- Louisiana Driver's License or I.D.
- Birth Certificate, passport, permanent resident card, VISA, or certificate of citizenship
- Social Security card(s) for all applying
- Most recent monthly income, such as: 4 paystubs, letters of employment (if you do not receive paystubs), Social Security award letter, proof of child support, proof of unemployment, or 2015 income taxes
